Once an AMVIR number is set up... An authorized representative may come with a AMVIR request typed on the company letterhead.
Required information:
- Each client search request needs to have two identifying markers, i.e. - Legal name; Date of birth; MVID and/or Driver's license number.
- Indicate who the information is being released to;
- State what section of the AMVIR that they are requesting under as well as the reason.
- Type of search is required - 3, 5, or 10 year personal or commercial drivers abstract.
The letter must also include the AMVIR number, be dated and signed by the authorized individual making the request.
